RACI stands for Responsible, Accountable, Consulted and Informed. These four key responsibilities can be assigned to tasks and activities to enhance communication.
RACI matrix are normally laid out as below.

Responsible
Anyone assigned with this role is responsible for completing the task or activity.
Accountable
Anyone assigned with this role is the owner of the work and will give sign-off to those responsible. There is only ever one accountable person.
Consulted
Anyone assigned with this role should be consulted on on decisions and provide input before the work is signed-off.
Informed
Anyone with this role should be informed of changes, progress and completion.
Formula in simple terms
Divide the total number of defects found by the total number of defect opportunities in the sample (number of units X number of defect opportunities per unit = total number of defect opportunities in the sample).
